The form and voided check may be submitted to us via email (info@fbcsfla.com) by sending us a photo or PDF of the documents. You may also drop-off the documents to the church.

FACILITY USE POLICY & REQUEST FORM
Use of FBCSF facilities for non-church sponsored events or activities requires prior approval by the church. If you need to request use of FBCSF facilities, please download and complete the attached "Facility Use Policy and Request Form." Your completed form should be turned into the church office for review at least two-weeks prior to the anticipated event or activities. No for-profit activities are allowed on the FBCSF campus.

Event Planning Guide
Use the Event Planning Guide to help plan all of the details of your event at FBCSF. We encourage all of our ministry teams and leaders to take advantage of this planning resource to help coordinate volunteers, communication, etc. related to events at FBCSF.

EVENT BUDGET TEMPLATEs
Use the Event Budget Template to plan out your event budget. All major events at FBCSF require an approved budget. The Budget Planning Template provides a simple guide for identifying expenditures and revenues for an event, helping to determine the anticipated budget for the event.

Couples Commitment FOrm
All couples who are married by a pastor at FBCSF must complete the Couple's Commitment Form and agree to pre-marriage counseling either with a pastor at FBCSF.

Wedding POlicy
All couples who would like to use FBCSF facilities for their wedding must complete the Facility Use Policy & Request Form that is listed above as well as read and sign the wedding policy of the church, which is attached below.

Liability Release Forms
For organizations utilizing the FBCSF campus and for individuals participating in certain FBCSF special events, the church requires that a liability release form be completed and notarized prior to participation.

CHurch Leadership Covenant
The Church Leadership Covenant takes leadership requirements and membership expectations from Scripture -- summarized in our Church By-Laws and Church Covenant -- and puts these into a format for volunteers, teachers, or other leaders to review and to formally covenant to in their service to the Lord through his church at FBCSF.
